Phase 1B of the COVID-19 Vaccine Distribution plan will begin soon.
We are asking Champaign County residents that are 65 years old or older and wish to receive a COVID-19 vaccine to complete a registration form either online or a printable version (both linked below). Once registration forms are returned to us, our staff will contact individuals to schedule their vaccination.
We have a drop box in the parking lot of the Champaign County Community Center that will be used for the printable registration forms. Residents are asked to complete the printable form and return it to the drop box or via mail.

Once you test positive for COVID-19, you may get a call from a 1-800 number with a recorded message from one of our staff members using a system called One Call Now. This call will give you instructions to visit our website and fill out our online contact tracing information form. This is a faster way for you to provide contact tracing information once you find out about your positive test results. This is not spam.
