Information Regarding COVID-19 Vaccine & Phase 1B Distribution Plan

Please note that we are only receiving limited doses of the COVID-19 vaccine. All you need to do is complete the form below. When we are ready to schedule you to receive your vaccine we will call you.

Please do not floor our lines with calls about scheduling.

Phase 1B of the COVID-19 Vaccine Distribution plan will begin soon.

We are asking Champaign County residents that are 65 years old or older and wish to receive a COVID-19 vaccine to complete a registration form either online or a printable version (both linked below). Once registration forms are returned to us, our staff will contact individuals to schedule their vaccination.

We have a drop box in the parking lot of the Champaign County Community Center that will be used for the printable registration forms. Residents are asked to complete the printable form and return it to the drop box or via mail.
