The Champaign County Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) is created through collaboration among many sectors of health and wellness throughout the county. The CHIP Leadership committee is comprised of members of the Champaign Health District management team, local hospital leadership, and the Chairs of the four CHIP committees: Mental Health, Healthy Living, Substance Abuse, and Early Childhood. By maintaining open communication and collaboration at monthly meetings, the CHIP Leadership committee ensures that all of the CHIP committees, the local medical community, and the health district are all working toward a common goal of increasing health in Champaign County.
